The Effects and Safety of Continuous Positive Airway Pressure in Children with Bronchiolitis: A Systematic Review and Meta-Analysis

Abstract Objectives To evaluate the effects and safety of continuous positive airway pressure (CPAP) for initial respiratory management bronchiolitis. Methods PubMed, EMBASE, Cochrane Library, CNKI, Wanfang data CQ VIP were searched until 17 June 2020. Randomized controlled trials (RCTs) which investigated effect CPAP bronchiolitis included. Results Twenty-seven RCTs met eligibility criteria. In comparison versus standard oxygen therapy, can reduce length stay (LOS) in hospital, rate (RR), PaCO2, heart rate, mechanical ventilation increase PaO2, but SpO2 PH not improved. high-flow nasal cannula, treatment failure, PICU LOS, Incidence intubation, RR, Modified Woods Clinical Asthma Score decreased. Treatment failure may be less helmet comparing with mask though there was no quantitative analysis. Conclusion significantly benefit children bronchiolitis, delivery by a better choice. More high-quality research is needed to confirm conclusion.
